Indemnification and Hold Harmless Agreement
The parent or guardian of _______________ agrees to indemnify and hold (Dancer’s Name Printed) harmless Rockledge Elementary/Intermediate School, Black Creek School, Emmanuel Lutheran church, United Methodist Church, Seymour School District and Seymour Dance Club’s teachers and all board members from all loss, damages, liability or claims arising out of
above named dancer participating in a dance class held at the Rockledge
Elementary/Intermediate School, Black Creek School, Emmanuel Lutheran church, or United
Methodist Church except to the extent same are caused by the negligence or misconduct of the
Rockledge Elementary/Intermediate School, Black Creek School, Seymour School District
Emmanuel Lutheran Church, and United Methodist Church. The parent or legal guardian of above named dancer agrees to handle, respond to, investigate and defend, at its sole expense, any claim or alleged claim made against Rockledge Elementary/Intermediate School, Black Creek School, Emmanuel Lutheran church, United Methodist Church, Seymour School District and Seymour Dance Club’s teachers and all board members arising out of the conduct of above named dancer participating in the dance class.

Rockledge Elementary/Intermediate School, Black Creek School, Emmanuel Lutheran church, United Methodist Church, Seymour School District and Seymour Dance Club’s teachers and all board members shall have the right to approve any legal counsel selected to defend the Rockledge Elementary/Intermediate School, Black Creek School, Emmanuel Lutheran church, United Methodist Church, Seymour School District and Seymour Dance Club’s teachers and all board members in such claim or alleged claim.
